Important Forms

For All Campers:  Please print out and complete the Registration and Emergency Form for each camper or Madatz (Junior Counselor) - these are needed in order to attend Camp Gilboa.

Emergency Form 

For A New Camper: Please print out and complete the Camper Information Form and page 1 of the Medical Form, with page 2 being completed by the camper's primary care doctor after a medical exam.

For A Returning Camper: If the Camper Information and Medical Forms were completed 24 months ago or more, please print out and complete them. Also, if there has been any change in the camper's situation, please send us completed forms.
